The purpose of gathering information is for internal records only. In the event that calls are received from our dispatch center, we will have record of the best way to contact you. The application is valid for one season. Each year a submission is required. Fire pits must be placed and used as per the respective hand outs attached above. It is the discretion of the Alpine Fire Protection District, if called out, to evaluate your fire pit during open operation, that weather conditions may require full extinguishment of the fire. The responding Captain will make that decision on scene. ***PLEASE INITIAL***

I own or legally control the above described land and agree to comply with all fire agency and Air Pollution Control District laws, ordinances, regulations, and terms of the permit. I understand if burning causes or creates a nuisance, the burn must be abated. I also agree to provide any information requested by the District.
